I, the undersigned, am the parent / legal guardian of the player(s) herewith registering.

I, the undersigned, hereby indemnify, absolve and hold harmless The Greek Sporting Club together with Olympiacos Soccer Club Johannesburg (both hereinafter referred to as “the Club”), its Lessor, Executive Committee, directors, shareholders, employees, coaches, agents, officers, representatives, sponsors, subsidiaries, subcontractors, contractors, affiliated third parties or staff member of the affiliated third parties and agree they will not be held responsible or liable for any claim or damage arising or suffered either directly or indirectly in any manner whatsoever to any person or member who accesses the premises, uses the facilities on the premises, competes for any teams on behalf of the club, is transported by the Club, or is coached on the premises, whether it be for personal injury or death or any harm caused to them or for damage, loss, theft or destruction of property arising from negligence or any other cause howsoever. The aforegoing will not limit or exempt any loss attributable to gross negligence or intentional wrongdoing. The terms of this clause are severable. Should any legislation restrict or prohibit the effect of this clause, such restrictions or prohibition will not invalidate the balance of this clause. I confirm that I have read, understand, and accept the Codes of Conduct and Rules and Regulations of The Club, and agree to abide by the conditions and rules contained therein.

I, the undersigned, hereby authorise OLYMPIACOS SOCCER CLUB JOHANNESBURG and the GREEK SPORTING CLUB through any of its representatives in the case of necessity, to act on my behalf in loco parent is and to give consent whether written or oral, for the performance of medical procedures, investigation and treatment of whatever nature which may be necessary for me, provided such operation, procedure, investigations or treatment is recommended and prescribed as necessary and urgent by a duty qualified medical practitioner.

I, the undersigned, further do hereby consent to the participation of any competition and sporting activity that may be held by the club or any other club and by virtue of such consent, I agree to submit to the rules, regulations and codes of conduct governing the club and/or competition. The club, its representatives, agents, or nominees reserve the right and unfettered discretion to change, alter and/or amend the rules, regulations and codes of conduct as may be from time to time.

I, the undersigned, herein warrant that I am duly authorised to accede to the terms and conditions contained herein above and that I am now fully aware of all the legal consequences pertaining thereto.

I, the undersigned, I understand that my child (under 18) might be photographed and/or video taped during their participation in activities and programs organised by the OLYMPIACOS SOCCER CLUB JOHANNESBURG and authorise such photographs and/or videotapes to be used by the club for club-related presentations.

I understand that my child's name will not be used and/or published in any way, and that no compensation will be given for the use of such photographs and/or videotape

I, the undersigned, I understand that should we no longer wish to participate in representing OLYMPIACOS SOCCER CLUB JOHANNESBURG, that we will not be entitled to a refund.
